Will coordinate:

• peripheral IV
• midline, PICC
• central venous access
• temporary dialysis access
• tunneled catheter (permcath, Hickman)

To request a consult:

1. Page 2VAS/2827

2. Provide:

   • Patient name, MRN, location
   • Type of procedure needed and clinical indication
     • if requesting dialysis access, confirm if patient also needs AVF/AVG
   • Patient's Cr, platelet, INR
   • Your contact information

3. Place order in Epic: IP CONSULT TO VASCULAR ACCESS SERVICE

   • Authorizing Physician: request attending name from Vascular Access Team
   • Provide your contact info, indication, brief history, procedure requested, and all other questions

4. If unable to reach pager 2827 after reasonable amount of time, OK to place order in Epic but MUST provide your contact info
